Learn how to submit your proof of vaccination or test results within Envoy

Overview

If your workplace has enabled the collection of vaccination or test result documents, you can submit them by using Envoy mobile or the web dashboard.

Mobile submission

✨ Important Note: Envoy only accepts JPEG or PNG file types as proof of vaccination. You will need to convert your file to one of these types in order to upload your document.

  1. Open up the Envoy Mobile app.

  2. Under the Home tab you should see the following prompt Submit your Vaccination Record.

  3. Click Get Started to begin the submission.

  4. You'll be redirected to another screen to add your document, select Get started.

  5. You'll be prompted to upload the front side of the picture.

    • You can either Take a picture of the proof of vaccination or upload the picture from your phone's gallery by clicking Choose existing picture.

  6. You'll also be prompted to upload the back side of the picture and if there's no back side, select the There is no back side option.

  7. You'll then be prompted to review your submission and add the date of the last dose administered.

    • You can retake any of the pictures by tapping on the side you would like to retake.

  8. Click Submit to upload the pictures to be reviewed by your Admin.

✨ Important Note:

If your submission is denied, you'll get the following message on the Envoy mobile app but you do have the option to resubmit.

Dashboard submission

  1. Log in to the Envoy dashboard.

  2. Click on your name in the bottom left and then Profile.

  3. Click on Vaccincations.

  4. Click Get Started or Resubmit Documents to begin the submission.

  5. You'll be prompted to upload the front side of the picture.

  6. You'll also be prompted to upload the back side of the picture and if there's no back side, select the "There is no back side" option.

  7. You'll then be prompted to review your submission.

    • You can re-upload any of the pictures by clicking edit photo.

  8. Click Submit to upload the pictures to be reviewed by your Admin.

Dashboard re-submission

  1. Log in to the Envoy dashboard.

  2. Click on your name in the bottom left and then Profile.

  3. Click vaccinations.

  4. Click Resubmit Documents to begin the submission.

  5. You'll be prompted to upload the front side of the picture.

  6. You'll also be prompted to upload the back side of the picture and if there's no back side, select the "There is no back side" option.

  7. Enter the date of the "last dose administered".

  8. You'll then be prompted to review your submission.

    • You can re-upload any of the pictures by clicking edit photo.

  9. Click Submit to upload the pictures to be reviewed by your Admin.

For California Users only

California users are able to upload their proof of vaccination using QR codes.

  1. Set up QR Code by going to this website, https://myvaccinerecord.cdph.ca.gov/.

  2. Take a screenshot or download an image of the QR code.

  3. Upload the screenshot/image on the Envoy mobile app or envoy dashboard.

Test results

Mobile submission

  1. Open up the Envoy Mobile app.

  2. Under the Home tab you should see the following prompt Submit Your Test Result.

  3. Click Get Started to begin the submission.

  4. You'll be redirected to another screen to submit your document, select Get Started.

  5. You'll be prompted to upload a copy of your test results:

    • You can either Take a picture of the test result or upload the picture from your phone's gallery by clicking Choose existing picture.

  6. You'll then be prompted to review your submission and enter Date issued.

    • You can retake the picture of the result by tapping the image

  7. Click Submit to upload the picture to be reviewed by your Admin.

Dashboard submission

  1. Log in to the Envoy dashboard

  2. Click on your name in the bottom left and then Profile.

  3. Click Test results.

  4. Click Get Started or Resubmit Documents to begin the submission.

  5. You'll be prompted to upload a photo.

  6. You'll then be prompted to review your submission.

    • You can re-upload any of the pictures by clicking edit photo

  7. Choose the date issued.

  8. Click Submit to upload the pictures to be reviewed by your Admin.
